Proof of Eligibility

To verify that you are a first responder, we will require the following: Law Enforcement Officers: Valid law enforcement ID card, paystub, Standard Form 50 (SF-50). U.S. government IDs are NOT accepted, such as TSA ID cards.
Firefighter

Must have active Proboard certification, must have U.S. Social Security Number, valid firefighter ID card, recent training completion certificate, a copy of a valid and professionally-issued certification illustrating firefighter status, or pay stub.

EMT/Paramedic

Valid EMT/Paramedic ID card, recent training completion certificate, a copy of a valid and government-issued certification illustrating EMT/Paramedic status, or pay stub.

911 Dispatcher
